Building Healthier Communities Through Evidence-Based Prevention
Health Promotion Project is a Minneapolis-based non-profit dedicated to closing health disparities across Hennepin County through culturally responsive wellness programming, free community screenings, chronic disease prevention education, and integrated social determinants of health navigation. Since 2011, we have served over 28,000 community members across 47 neighborhood sites.
Measurable Health Outcomes Across Hennepin County
Our programs are rigorously evaluated using pre- and post-intervention health assessments, biometric tracking, and longitudinal follow-up studies aligned with Healthy People 2030 objectives.
Evidence-Based Health Promotion Programs
Every program is grounded in peer-reviewed public health research and tailored to the specific health challenges facing our community. All core programs are available free of charge through Title V and HRSA grant funding.
Healthy Foundations
Our flagship 12-week chronic disease prevention series covers cardiovascular health, diabetes risk reduction, nutrition fundamentals, physical activity integration, and stress management. Developed in partnership with the University of Minnesota School of Public Health and delivered at 12 community sites across Minneapolis, this evidence-based curriculum has demonstrated a 23% improvement in participant cardiometabolic risk profiles at 6-month follow-up.
Learn More →Freedom from Tobacco
An 8-week comprehensive tobacco and nicotine cessation program combining cognitive behavioral therapy techniques, motivational interviewing, peer support circles, and nicotine replacement therapy navigation. Our program follows the U.S. Public Health Service Clinical Practice Guideline for Treating Tobacco Use and Dependence. Participants receive free nicotine patches, gum, or lozenges through our pharmacy partner network.
Learn More →Diabetes Prevention Program
A CDC-recognized 12-month lifestyle change program for adults at high risk for type 2 diabetes. The curriculum includes 16 core sessions covering healthy eating, physical activity goal-setting, and problem-solving barriers, followed by 6 monthly maintenance sessions. Stories from Our Participants
“The Healthy Foundations program completely changed how I think about food and movement. My A1C dropped from 6.3 to 5.7 in four months. The community health workers spoke Somali, which made everything so much more accessible for my family. I cannot say enough about this organization.”
Amina H., Cedar-Riverside
Healthy Foundations Graduate, 2024
“I had been smoking for 22 years and tried quitting on my own at least a dozen times. The Freedom from Tobacco program gave me the tools, the accountability, and the free nicotine patches I needed. I have been smoke-free for 14 months now. They saved my life, truly.”
Marcus T., North Minneapolis
Freedom from Tobacco Graduate, 2023
“As a school nurse, I refer families to Health Promotion Project regularly. Their community health workers bridge the gap between clinical recommendations and real-life barriers like transportation, language, and insurance. The screening events they hold at our school have caught early-stage hypertension in at least three parents that I know of.”
Jennifer K., RN, BSN
Whittier Elementary School Nurse
